Ask an AI note-taker to separate proposals, confirmed decisions and unresolved questions. A polished summary is not useful if it turns “we could launch next week” into permission to launch. Keep the condition, the responsible person and the evidence for the interpretation beside the recorded choice.

Transcription accuracy is only the starting point. Even correctly captured words can be summarized into the wrong commitment. The decision record below is designed for the person who must act on the meeting afterward.

Use one record for each decision or unresolved issue. Empty fields are useful evidence: if the owner or deadline was not agreed, leave it unassigned rather than having the agent invent a plausible value.

The June 2023 W3C Process document is a useful institutional example: its meeting-minutes section calls for official decisions to be recorded while their rationale remains clear. We cite that dated process as an example, not as current meeting rules for every organization.

The W3C PROV primer describes relationships among information, producing activities and responsible people. That supports traceability as a concept. Our meeting fields and review procedure are a practical adaptation, not a format prescribed by the primer.

Consider a hypothetical meeting in which the team chooses Friday for a release only if the accessibility review is complete. A summary that lists “Release on Friday” has removed the condition that makes the decision valid. The record should keep the date and prerequisite together.

If the meeting assigns a person to arrange the review, that assignment is separate from permission to release. The agent should preserve both entries without assuming that the reviewer owns publication or that scheduling the review means it passed.

If the prerequisite is not met, the record should not quietly invent a new release date. Mark the affected choice as unresolved under the original condition and identify who can decide the next step. A compact record can preserve this distinction without reproducing the entire discussion.

A review request should show the proposed record and the relevant source location. Ask whether the choice, status, condition and ownership match what was agreed. That is easier to correct than a general request to approve a long meeting summary.

Silence is not a substitute for confirmation when the decision remains ambiguous. Nor does a confident statement from one participant necessarily establish the organization’s authorization. Apply the team’s existing decision rules and record the person whose confirmation resolves the uncertainty.

If the recording is unavailable or the relevant passage is missing, label that limitation. The person responsible may still confirm the decision from their own knowledge, but record that confirmation as a separate source. Do not pretend the agent recovered words it never inspected.

When a subsequent meeting changes a choice, create a dated revision that identifies what it replaces and why. Keep the earlier record available where appropriate. Otherwise a reader may be unable to tell whether an action followed the decision that existed at the time.

Link an action to the applicable confirmed record before an agent creates tasks, sends messages or updates shared systems. Summary generation and execution should have separate authorization boundaries. An automatically generated owner or deadline must not become an instruction merely because it appeared in a structured field.
