None of them generate the applause that an agent shipping to production does, and so they get deferred. The artefacts exist; the enforcement does not; the gap between the two is where incidents live.

Stage 8 exists to close that gap. The six templates in this kit are designed around a single principle: every artefact pairs a written rule with a mechanism that makes the rule operative. The charter names decision rights and the escalation path that fires when those rights are challenged. The risk register names a quarterly walk-through that forces every entry to be re-justified or retired. The model-update template names eval gates that block production rollout. Without those mechanisms the documents are governance theatre — useful for an auditor, useless against an incident.

The other reason Stage 8 deserves its own kit is that governance is the stage with the weakest demo. You cannot screen-record a governance charter the way you can screen-record an agent successfully completing a task. Governance work compounds silently and visibly fails loudly, which is exactly the cost profile that under-rewards investment until an incident inverts the calculation. The win condition for Stage 8 is not a perfect charter — it is a complete kit that bites. A modest charter paired with a live register, a rehearsed incident runbook, and a quarterly audit that produces decisions beats a beautiful charter that lives in a drawer. Optimise for the operating loop, not the artefact polish.

02CharterCommittee, decision rights, escalation.

The governance charter is the load-bearing artefact. It names the committee that owns AI governance, the decision rights that committee holds, and the escalation path that fires when those decisions are contested. Get the charter wrong and the rest of Stage 8 has no anchor; every other template assumes the charter already exists and answers questions the charter is supposed to settle.

Three structural decisions matter more than the rest. First, the committee composition — small enough to decide, broad enough to see the surface area. Five to seven members typically: an engineering lead, a security or risk representative, a legal or compliance representative, a product or business owner, and an executive sponsor. Second, the decision rights — which decisions the committee makes directly, which it advises on, and which it delegates. Third, the escalation path — what happens when the committee cannot agree, or when a decision is made outside it and someone wants to challenge.

The template below is a stripped-down YAML expression of the charter contract. Teams almost never use the YAML in production — the operating doc is usually a Confluence page or a Notion workspace — but writing it as a contract first forces the structural decisions to be explicit before the prose softens them.

Two operational notes on the charter. The chair rotation matters more than teams expect — eighteen months is long enough for the chair to learn the role and short enough that no single person becomes the de facto definition of governance. Second, the executive sponsor is a separate role from the chair, by design. The chair runs the committee; the sponsor escalates when the committee is stuck. Conflating the two roles is the single most common charter mistake we see in audits.

03Risk RegisterCategorised by severity and likelihood.

The risk register is the second load-bearing artefact. It converts "we worry about AI risks" into a structured, scored, owned list that the committee can walk through in a single ninety-minute meeting. Without the register, every governance discussion starts from zero — what are we worried about, how worried, who owns it — and that re-litigation is where most committees lose their afternoon.

Eight risk categories cover the canonical surface area for agentic AI systems: data leakage, model failure, vendor concentration, supply-chain (training data and tool chain), bias and fairness, regulatory exposure, operational dependency (what happens when the model is down), and reputational. Each entry in the register names the category, the specific risk statement, the severity (Low / Medium / High / Critical), the likelihood (Rare / Unlikely / Possible / Likely / Almost certain), the current mitigation, the residual risk after mitigation, the owner, and the review date.

Not a team, not a role — one person. The owner is responsible for re-reviewing the entry at its cadence, proposing changes to severity or likelihood, and reporting on mitigation health. Shared ownership is the most common register failure mode; entries with no named human end up not reviewed at all.

The committee walks every entry in the top-right quadrant monthly, the top-left and bottom-right quadrants quarterly, the bottom-left annually. The cadence concentrates attention proportional to risk-weighted impact. Walking the entire register every month produces fatigue and review skipping.

The discipline that separates a live register from a dead one is the monthly walk-through producing decisions, not just status updates. A register meeting that ends with "no changes" is a meeting that wasted ninety minutes of the committee's time; a register meeting that ends with three entries re-scored, one entry retired, and a new entry added is governance doing its job. Train the chair to ask "what decision did we just make?" at the end of every entry walk.

Worth flagging — the register is one of the strongest cross-links to the vibe-coding policy audit. Several of the audit's 50 points map directly to register entries — IP exposure, shadow-AI risk, secrets leakage. Running the audit and updating the register on the same quarterly cycle produces compounding governance with barely any incremental effort.

Annual audits ratify outcomes that already happened. By the time an annual audit finds a problem, the problem has been operating for an average of six months — long enough for drift to compound into cost, embarrassment, or regulatory exposure. The Stage 8 cadence replaces the annual ceremony with three nested rhythms: weekly for production health, monthly for governance artefacts, and quarterly for the policies and frameworks themselves.

Each cadence has a different audience, a different deliverable, and a different escalation path. The weekly is engineering-led and surfaces operational issues; the monthly is committee-led and surfaces governance-artefact issues; the quarterly is executive-led and surfaces policy and framework issues. Strategic rhythm

The cadence that gets dropped first under pressure is the weekly. Resist that. The weekly is the rhythm that catches drift before it becomes the monthly committee's problem or the quarterly executive's problem. Cancelling the weekly to free engineering time is a false economy — the time saved comes back as monthly committee thrash or as a postmortem the quarterly never anticipated.

The cadence that gets dropped second is the quarterly. The argument usually sounds like "we already audit monthly, we don't need a separate quarterly," which conflates two different review depths. The monthly walks artefacts; the quarterly walks the frameworks that produced the artefacts. Skipping the quarterly means policy and framework drift goes unreviewed for a year, which is what annual audits were designed to fix and what Stage 8 is designed to replace.

05Model UpdatesEval before rollout, rollback gate, communication.

Model updates are the highest-frequency governance event in an agentic AI program. Vendors ship new versions every few weeks. Each release is a potential improvement and a potential regression — sometimes both in the same release. The model-update review template names four gates that have to clear before a production swap: eval, canary, rollback readiness, and stakeholder communication.

The template below is the YAML contract that engineering teams translate into a runbook or a CI workflow. The point is not the file format — it is that every model update follows the same four-gate sequence so that nothing slips through on a vendor announcement alone. The most common failure mode in this space is silent adoption: an engineer updates a model identifier in production code, the change merges through ordinary review, and no governance gate ever fires because none of them were wired into the process.

One subtlety worth pulling out: the rollback authority is broad on purpose. Restricting rollback to the committee creates a governance bottleneck precisely when speed matters. The template grants any of three roles the authority to roll back without committee re-approval, with post-fact ratification at the next committee meeting. The asymmetry — fast to roll back, slow to roll forward — is the point.

The eval gate is the gate teams fail most often. The standard anti-pattern is "the vendor said it benchmarks better, so we shipped it." Vendor benchmarks are useful signal but they are not your eval suite. Your eval suite measures performance on your tasks against your baseline, and that is the only measurement that gates a production swap.

06IncidentsDetection, containment, communication, postmortem.

The incident runbook is the artefact teams hope never to need and inevitably do. Four phases structure the runbook: detection (how the team learns of the incident), containment (how the blast radius is reduced), communication (who is told what and when), and postmortem (how the lessons feed back into the rest of the kit). Each phase has a named owner, a target time-to- execute, and an artefact that proves the phase ran.

Severity classification is the structural decision that shapes the rest of the runbook. Four tiers — SEV-1 through SEV-4 — cover the canonical surface area. SEV-1 is customer-facing harm or regulatory exposure; SEV-2 is significant degradation without customer harm; SEV-3 is contained operational disruption; SEV-4 is near-miss or anomaly that warrants recording. The committee's emergency-convene threshold is SEV-1 or SEV-2; SEV-3 and SEV-4 roll up via the monthly cadence.

The runbook is only as good as the last rehearsal. The Stage 8 kit treats incident runbook rehearsal as a quarterly cadence item — a tabletop exercise where the committee walks through a simulated SEV-1 or SEV-2 scenario and tests every phase of the runbook. The rehearsal is where you discover that the communication template references a Slack channel that no longer exists, the rollback runbook references a service that was renamed, the on-call escalation list contains a former employee. Rehearsing once a quarter catches all of that before the real incident does.

A note on the "AI did it" failure mode in postmortems. The model is never the root cause of an incident — the human decisions that allowed the model output to ship without an adequate gate are the root cause. Stopping postmortem analysis at "the model hallucinated" produces no preventive action and erodes confidence in the tooling. Every postmortem walks the decision chain backward until it finds the gates that should have caught the problem and proposes the changes that make catching it automatic.

The pattern that catches teams
The runbook gap that costs the most in real incidents is the communication template. Engineering teams over-invest in detection and containment and under-invest in the holding statement, the customer notice, and the partner email. When the incident hits, the communication scramble takes longer than the containment did. Pre-write the templates; the worst time to draft customer-facing language is at 2 a.m. while a SEV-1 is open.

07EthicsDecision framework for gray-area use cases.

The ethics-review template is the artefact that most teams either skip entirely or reduce to a checklist that nobody consults. Both failure modes miss the point. Ethics review is a forum, not a checkbox — the value is in the structured conversation between people who do not agree, conducted before a decision is made, on a use case where the right answer is not obvious. Reducing the forum to a signed checklist eliminates the disagreement that produces the value.

The use cases that warrant ethics review share a pattern: disparate impact possible, vulnerable populations affected, persuasion at scale, surveillance or scoring of individuals, synthetic media generation, automation of consequential decisions. When any of those triggers fire, the use case routes to the ethics forum before the implementation work starts. Routing after implementation is the standard failure mode — the forum is convened to review a system that has already been built, at which point the conversation is about justification rather than design.

Either party can request a re-review if new information emerges. Re-reviews are conducted by an overlapping but not identical forum to reduce anchoring. The appeals path matters because forum decisions made early in a program will be tested as the program scales — the path needs to exist before it is needed.

Two-stage appeals

Two operational notes on the ethics forum. First, the forum chair is a separate role from the governance-committee chair, by design. The skills overlap but the dynamics do not — the committee chair runs decisions toward closure, the forum chair runs disagreement toward productive depth. Conflating the two roles tends to produce committees that close too fast and forums that close too slow. Second, the forum's output is an artefact the wider organisation can read. Confidentiality often gets cited as a reason to keep the discussion private, but the discussion can be private and the decision can be public — and the public decision is what builds the precedent.

The hand-off matters because governance models that survive a single business unit often fracture when applied to three. The Stage 9 templates assume the Stage 8 artefacts exist and answer questions the Stage 8 kit is supposed to settle — committee composition, decision rights, register methodology, audit cadence, model-update process, incident runbook, ethics forum. Without the Stage 8 output, Stage 9 has nothing to scale.

The trap in Stage 8 is treating the charter as the deliverable. The charter is a Confluence page. The deliverable is an operating loop in which a named committee makes named decisions on a named cadence, a live risk register surfaces the real surface area, audits at three rhythms catch drift before it compounds, model updates pass four gates before they ship, incidents follow a rehearsed runbook, and an ethics forum convenes early enough to shape design.
